Dickson’s History of the Theory of Numbers has two non-overlapping indices: an Author Index and a Subject Index. Having used that book — and referred to both indices — literally thousands of times in my life, I can honestly say that if they were in a single index, differentiated only by fonts, it would have been a far more frustrating experience.
