I had that problem with one system, it always occurred in the late afternoon.
After running many tests, I suspected something might be shorting out an extension line. They were using many existing wire runs in their building. All the digital phones were working ok, so I suspected one of the analog extensions pairs. Since they were not using any slt's on the system, I went through and disconnected all analog pairs from every extension (of the old wiring, I trusted my new wiring). - on the punch blocks, that would be the first pair (blue/white) of each extension port. Once I did that the problem disappeared and never returned. They moved from that building about a year later so I never found out what the problem really was.
