For the benefit of others who might encounter similar problems, the 2xx entry in the TVS 'extension numbering plan' table appears to have been cleared somehow (this did work at one time...). The only entry was 1xx, so any leading digit other than 1 was simply passed to the menu system. Restoring the 2xx entry cured the problem.
