I can't compete for the oldest post, but I can chime in on the Outlook problem. 
 I've had several 
people lately with that problem, and its extremely nasty to recover from.  Its 
a good reason to 
migrate to a real email client like Thunderbird that doesn't have problems like 
this, or sending out 
invalid return receipts (like LookOut does).
