My guess would be either a bad extension card or a cable\telephone short on  
one of the extensions.  
 
What are the ESLC's being used for?
 
Any new phones or ext's added recently before the problem started?
 
Next time it happens try this.  While the system is experiencing the  
problem, disconnect the amp cables one at a time and then test (make incoming  
call). 
 If its a cable problem, once you remove the amp for the bad ext it  should 
clear up.  If a cable or phone is shorting the ring signal to ground  even 
slightly this can cause problems.  We used to see this on the 61610's  where 8 
of 
the 16 ext's would stop working.  Unplug the shorted cable and  problem 
clears.  
 
Be sure that you don't have any SLT's connected to the DHLC card with all 4  
wires to the jack.  Many slt's can short or put a load on the data pair  
effecting the power supply.
