Check what happens when you call to the door while it is locked up. We just had a service call to a day care center with a 123211D that has had their doorphone lock up frequently. This time when I called it the display said Busy. Their DSS console showed a cordless phone off the hook (a phone they rarely use). The cordless had answered the door (possible by being knocked out of it's charger during a door ring). If I didn't see that on the DSS, the simplest solution would have been to reboot the system rather than check all 28 extensions.
Other things related could be something or someone dialing 311 or 312 by mistake then somehow locking it up.
