If there is an existing feed, use it. You don't need cat 5. After all,
it's probably being carried on 50 year old cable for 2 miles to your
building, another hundred feet of cat 1 is insignificant.

To answer your question, if you wire it up like an ethernet cable
(straight-thru), it'll work just fine. If you want to use existing
cable, you need 2 pair, and it'll use pins 1&2 and 4&5.

This is a seemingly simple question, just want to be sure.

I am running a cat5 line for a T1 drop from the T1 circuit that is
terminated in the closet, to a KXTD1232 with a T1 card. The service
provider is AT&T.

Question: Is it safe to terminate the cat5 run with RJ45 connectors and
patch cords, using the T568B standard? Or do I need to make a crossover
termination??
