Does the phrase "This Feature Is Not Used in the U.S.A." mean anything to you all? Our old Step-By-Step central office reversed the supervision (Polarity of T&R) when the call was answered. You can get that for COCOT lines in some places and you pay dearly for it today. I guess some countries still use it as a method of determining an answered call.
