Finally, I get to contribute something!  I figured it out.  The 1232
lights up a CO button programmed as a MWL for another extension based
on the MWL of that extension; it apparently has nothing to do with the
VPS 50.  To have a CO button programmed as a MWL for a phantom
extension which has been assigned to an extension group in the VPS,
simply pick an extension in that group, not the phantom.

For example, assume you create a phantom extension 295 in the 1232,
and you assign that extension to an extension group in the VPS50
consisting of extensions 113, 115, 118, 125.  You then create a custom
service which sends the caller to voice mail for extension 295.  When
the caller leaves a message, the MWL's on 113, 115, 118, and 125 all
light up.  If you want to program a CO key on, say extension 101 to
light up as well, you program it to be a MWL on extension 113, NOT
295.
