To zoom in you probably need to press ctrl+shift+= (note that to get +, you
need to press shift+= after all). You can change it in Keyboard Shortcuts.
There are also shortcuts = and - (zoom in and zoom out) that don't need you to
think whether you need to press shift or not.

I'm not exactly sure why it happens. What keyboard layout do you have?

For the "fit to window", you can press "2" or "3" to get different "Fit to..."
options.
