Do you see the same thing with the default Breeze icon theme? I suspect this is
only seen with 3rd-party icon themes, resulting from a quirt of the icon
loading mechanism whereby a smaller icon from theme theme is sometimes
preferred, and then it gets upscaled and becomes blurry. See Bug 501500.
If the issue goes away when using the default Breeze icon theme, it's Bug
501500.
