Review Comment:
`orElse("null")` will log the literal string `"null"` when no instance id is
set. If this case is reachable, something like `orElse("<not set>")` is
clearer, but actually if we're handling `FENCED_INSTANCE_ID` we must have sent
an instance id — so this can't be empty. Could just use `get()` here, or assert
the invariant.
