And for ext/mysqli which supports OO, studlyCaps would make life harder, to migrate "old" php4 scripts to PHP5 and ext/mysqli.
How about an additional configure option --with-studly-caps (disabled by default) and defining some additional aliases?
No matter what the decision, I definitely don't want any configure options and no INI options.
