Any clues what might cause this or what I can do to rectify it?


Yep; I just hit this:

6914346 upgrade from OpenSolaris 2009.06 (111b2) to 130 fails with stale archive_cache

Workaround is (from the 2009.06 BE):

beadm mount <target_be> /mnt
bootadm update-archive -F -R /mnt
bootadm umount <target_be>
