I have no idea, but one approach is to find an old manual for the product
(bitsavers?), and look at the inside front cover where there is usually an
address to send comments to if the form at the back is missing.

Typically the postage-paid form at the back has a generic US address, but
the address at the front is of the location where the product was developed
and/or is supported from.

Another clue can be the "form number" (manual number), Although
undocumented, the 3rd and 4th characters (normally numeric) are often
related to the issuing location. I don't have a list, but if you find a
book that is (made-up example) -  GC12-3456 - then look for other books
that have 12 in those same positions, and see what they're about.
