Larry, It appears as though somehow PTF UA60411 was removed from your PTS outside SMP/E's control. It could have been removed by a REJECT SELECT, but that would then show it wasn't in the GLOBAL zone.
Would it work to try the RECEIVE again, but do a BYPASS(APPLYCHECK)? I don't know if this would work, being that your system is showing the PTF as RECEIVEd and APPLYd? If that doesn't work, you could try doing a REJECT of the PTF with a BYPASS(APPLYCHECK) to get it out of the GLOBAL zone, followed by a RECEIVE with BYPASS(APPLYCHECK) to get it back.
