Try emptying the file first (edit_defaults) and reconstruct the entire file.
This should solve the issue you are running into.

It seems quite obvious that some configuration files require a determined
ordering in order to function correctly. I could work around it, but that
would be a very tedious task, defining a relative location for every line.
insert_type => literal sounds like the right option, but doesn't function as
