We had a discussion on this topic some time ago on this list, see https://lists.gnu.org/archive/html/groff/2007-08/msg00115.html The problem is illustrated in the image attached to that message. Adobe's Courier and Microsoft's Verdana and Tahoma all have this problem, IBM's Courier is an exception by using a different style of quotes (which IBM also continues to use in Plex).
