I would say that a custom control is the way you should follow. However you don't have to create the full control element. Just a single invisible 18px square <div> would do to pick up the clicks and to provide cursor change and a tooltip title. Positioning may be tricky. I have a feeling that there may be some small variations in control positioning between browsers.
