Clicking on Edit: Tax Report Options
The first entry for Sched D has:  Dividend, cap gain dist
on my screen in a small popup.  So, I was mislead by the word
'Dividend' in that entry.

You are correct, they should be Sched B: Total dividend income

Is it possible to have two entries:
  Qualified dividends
  Ordinary dividends
