I have no problem with sound output from Firefox compiled from source. I do
not use pulseaudio, only pipewire with whatever might be its default settings,
on a Plasma desktop. These are my USE flags:

Some things to check:
1. Do you have sound when you use a media player? If yes, we can assume your
system drivers, desktop and hardware is working as it should and the cause of
the problem is with Firefox itself.
2. Have you checked if for some reason Kmix is muted for Firefox only?
3. Are all audio codecs, streams, websites failing to play, or only some?
4. Have you changed the default Permissions in Firefox? Did you try launching
it with a new profile?
Take a look at settings under: Edit > Settings > Privacy & Security >
then look at the settings for 'Speaker Selection' and 'Autoplay'. By default
the 'Speaker Selection' is empty and 'Autoplay' is set to 'Block Audio' for
all websites.
Hopefully, one of the above may yield something useful.
