CHKDSK and DEFRAG ain't that reliable really. Regarding SCANDISK, maybe you used an old version (MS-DOS 6.x) on a hard disk that's addressed using LBA? If your hard disk is larger than 8 GiB and the partition is partly or fully outside the first 8 GiB of the disk, then that might lead older SCANDISKs to work improperly.
