Thank you for the interest in the form! To answer a few of the questions:

- The form populates a pending patron in the pending patron interface.

- The uploaded images are automatically deleted after a set period of time.

- We had a great deal of discussion about the fact that we have the digital images of the ID, and we consulted with our City's lawyer about this. In the end, we went forward with it, recognizing that patron interaction with the form is a type of implied consent for us having access to these images. We only keep the images for a set period of time, and there are two levels of security for staff to be able to access them. We also offer other options for obtaining a library card if a patron is uncomfortable uploading their images with us.

        One of the nice features of the form is that it allows a
        co-signer to register for multiple juvenile cards at once. It

                    schema of the database to add this piece of data

                    This is surprising to us.  We are not a public
                    library, and have had few requests to register
                    juveniles so far.  But since Evergreen was
                    designed for public libraries, we were quite
                    surprised to find that we can't easily add the
                    Parent/Guardian field to the OPAC registration form.


                    I checked Launchpad and it looks like it has
                    already been listed as a bug
                    (https://bugs.launchpad.net/evergreen/+bug/1897103),
                    although there's not much heat on it yet.  Is
                    this really not a problem for most public
                    libraries?  How do you all deal with the
                    problem?  When registering juveniles, do they all
                    have to use an offline form to register?
