Are you referring to the US?  Or to what nation?  

I don't know about other EU countries, but in France, 3-phase power is 
available to (AFAIK) every house. It costs more, though, and not everyone is 
willing to pay.

Possibly because, at least in France and Spain, the larger your capacity, 
the higher the standing charge you pay.  

In France, 6kva is common, with 12kva considered generous.  In Spain, where 
I think the cost difference is greater, lots of folks get by with 3kva 
service.

If you use too many appliances, the main trips. You turn something off and 
reset it.  No big deal.

A little cafe that we hang out in regularly trips their main every now and 
again.  None of the customers is surprised or bothered.  The power is back 
in a couple of minutes.  Meanwhile, they have battery powered lamps on all 
the tables.  :-)
