This indicates either a bad 5I20 card or perhaps dirty PCI contact fingers/edge connector contacts. I would try moving the 5I20 to another slot if possible and in any case, cleaning the contact fingers on the 5I20 and the edge card connectors on the PC with 99% Isopropyl alcohol, and try again. Most 5I20 problems that are not contact finger related are either damaged FPGA chip (unlikely unless you connect directly to the FPGA pins without a daughtercard) and occasionally leaky EEPROMS. Leaky EEPROMs can be fixed by running the EEPROM setup batch file, this unfortunately requires DOS.
