It's commonly called a "repstrap" (like RepRap), and people have
been doing it at least as long as regular 3d desktop printers.  Building
your filament extruder into a tool holder has somewhat limited usefulness,
since you aren't going to be able to feed the filament through the spindle,
or attach the filament spool to the tool holder.  This certainly precludes
using it in a tool changer.  If you use a CAT40 tool holder as a convenient
attachment point for an extruder, you need to have spindle orientation
and spindle lock to it can't rotate while you are printing.  As Andy suggested,
most CNC milling machines are much, much slower than you would like
for a printer because of all the mass they have to move .  Mori Seiki has a
neat new 3d printing attachment (using powdered metal, not plastic filament)
 for their five axis machining centers, but even they don't try to mount it in 
a tool holder.  It attaches to the side of the spindle housing.

It ought to work, but the typical 3D printer moves a lot faster than a
milling machine.
