Makerslide, Open Rail and various other "open" designs are 20 mm. I'm working 
with 40 mm extrusion.

What would work is double edged V rail wide enough to work on 40 mm extrusion 
with a single slot on each side. Found a lot of single edge V rail but can only 
bolt one to each side unless you don't need the edges in the same plane.

Open Rail solves that issue, but doesn't have a 40 mm version. It's slotted 
holes allow fitting to 20 or 25 mm extrusion.
