Have you tried a test user with local authentication?

Dovecot currently does blocking lookups, so if ntlm_auth is slow on responding (e.g. network problems), Dovecot blocks all other authentication requests until it's finished. Just wondering if Outlook parallelises it's sessions and ntlm_auth is blocking because of the timing between the requests.

Or perhaps try

auth_cache_size = 1024

To cache authentications.
