The '+' flag cannot be used in the main error log format, but the explanaiton
in the doc(http://httpd.apache.org/docs/2.4/mod/core.html#errorlogformat)
doesn't mention it. So it is better to add it after the paragraph
"The above behavior can be changed by adding modifiers to the format string
item. ... . If an item with the plus modifier does not produce any output, the
whole line is omitted."
