In the document of directive "IndexOptions"
(http://httpd.apache.org/docs/2.4/mod/mod_autoindex.html#indexoptions), it
doesn't mentioned the conflict between '+/-' modifier and those option values.
For example, the '-' cannot combine with 'IconWidth=n'. So it is better to add
the confliction information.
Also, the handling logic about 'IndexOptions NameWidth' indicates that the
NameWidth value must be greater than 5, however, the document doesn't mean
this.
