I just checked the full-book ODT from which the PDF was created. As expected, it has all the same formatting errors, so that is evidence of some problems in the ODM. (I can't think of what else might be causing these anomalies. I don't see the ODM in the "for wiki" folder, so I assume it's the one in the previous folder. If I have time, I'll look at it later.
