For a very small office -- 10 or so -- an outsourced VOIP PBX is probably a good deal. It's certainly what you want for 1-6 people. For larger sites, I think buying "trunk" SIP or IAX2 on a minutes-used basis becomes cost effective. Certainly it does for my office of 30. I run two Asterisk servers: the primary is at the office, and handles voice calls and conferencing; the secondary is at a data center, and currently only handles voice mail and voice calls when the primary is down.
