I think the exisiting UI could be used as follows:
- file the addendum using the same form, but append a suffix (e.g. yyyymmdd) to 
the file name.

I'm not convinced that it's worth providing a separate process.
Nor do I think it's worth changing the layout of the CCLA directory to group 
related documents in their own directories.

Unlike ICLAs, which are frequently referenced, AFAICT CCLAs are rarely 
referenced once filed.
