"Bold" must be pretty new. To me it's always been "Bright", and that's how the colour escape codes are defined, and the CGA display with four bits for Red/Green/Blue/Intensity. If you can't show the bright colours correctly due to a lack of colour registers or bitplanes, you have to resort to other methods of making the letters stand out a little more, for example making them bolder.
