Proposal:
If group.main=true then the first grouping command will be used for the main
result list (i.e. what you currently get for a normal ungrouped response).
For this main result list, the parameters will be interpreted somewhat
differently:
- group.offset will be ignored... it doesn't make sense in a flattened list.
- start/rows will revert to their traditional meaning - numbers of documents,
not numbers of groups
