[~stoty] [~michael-o]  For the sake of consistency with the existing auth 
schemes it would be great that the new scheme

1. throws an exception in case of a protocol violation / misconfiguration / 
unexpected context

2. propagates 401status  in case of authentication failure (bad credentials) 
and if necessary populates additional details in the `HttpContext`
