When the flash drive is inserted, kernel modules are modprobed and hal
gets fired up, causing the icon to appear.  Evidence: nothing in
.xsession-errors, happy kernel module activity in /var/log/messages, and
happy kernel modules and hal activity in /var/log/syslog, which is
attached as "syslog" to this.  "mount" does NOT show the drive as
mounted.

gvfs-mount was not present on my system.  Synaptic could not find it in
any repository.  I downloaded from:

https://launchpad.net/ubuntu/hardy/amd64/gvfs-bin

the raw .deb file and installed it with dpkg, which worked.  It appears
gvfs-bin is having a colorful history on amd64?

I have to continue in another addition, it appears only one attachment
per comment?
