Why I don't know, but:
Instead of 40_custom, I use 41_custom, but copied to 07_custom. Grub.cfg then
reads custom entries from /boot/grub/custom.cfg at runtime, placing them ahead
of
its own entries. Any changes you make to /boot/grub/custom.cfg are read without
any additional fuss next boot and beyond, until you change it again.
