The apt/dpkg databases concern themselves only with: installed kernel-source packages which are zipped tarballs, installed kernel-image packages that you downloaded, or built using kernel-package. Whether and where you unpack the tarballs, patch them, build them, etc. is up to you, does not have to be done as root (use fakeroot), and does not trouble apt one whit.
