Here's what I'm using in .fetchmailrc (it works for me, at least):

poll <isp_adress> proto pop3 user <username> password <password> mda 
"/usr/bin/procmail -d%T" limit 200000

I've also gotten it to work with /usr/bin/formail as MDA.  exim and
smail are MTAs, not MDAs.
