Not excessively difficult with any of the most-used MTAs, in any event. Some (Postfix, Qmail, Exim) require patching/extensions or a prepatched package. Some (Sendmail, Courier-MTA) do not. But you need to make or get and then sign appropriately an SSL cert, for any MTA -- and do necessary MTA configuration work.
