I've just adopted fvwm2 and am looking into this problem now. I'll know more about the first problem soon, but in order for me to be able to deal with the second one, I would really appreciate seeing an old hook to understand how /usr/lib/X11/fvwm2 is needed/used so that I can handle it properly. BTW, would all of these default handles be living in /etc/X11/fvwm2?
