Well, I do not know if your system is a special case, but I suspect this need to be solved in the libdevmapper package. Fixing it in insserv would be hard. I added code in insserv to refuse to enable it while the system had obsolete init.d scripts around because I discovered problems with scripts with wrong header that was already removed from their package, and thus it was impossible to get the scripts fixed. The correct fix as far as I know is to get the package maintainer to remove these scripts during upgrades.
