It seems that linux-boot-prober must be getting confused and reporting
that /boot is on a different partition. Can you boot d-i and, after the
hard drive modules are loaded, run the command:
  linux-boot-prober /dev/discs/disc0/part5
and send me the output. Feel free to include the output when it's run on
all your other linux partitons too; the output of the os-prober command
wouldn't hurt either.
