Sounds like the card is defective... If you got the cheapy from Wal- Mart, they'll take it back within 90 days (at least they used too, policies always change)... I bought that same card and the first one was dead, they let me exchange it and I never had a problem with the second one, it's in my other system...
I think you've done all the troubleshooting you can do with that one though, I don't think it's windows, or your mobo... I do have to ask though, does it get real hot in the case? If hotter than usual, you may burn up the next one, maybe get some RAM heatsinks to put on the chip or something, the little ones with adhesive backing...
