Sorry for the late reply.
I looked at your yaml configuration, and nothing looks suspicious. The real
root cause fix would take some time because it is extremely complicated. In the
meantime, I would say do the following two workarounds:
1. Is there a specific reason behind keeping the duration too short, 180000ms?
Is it possible to increase?

permissions_validity
roles_validity {code}
2. Have you tried increasing {_}memtable_heap_space{_}?
