My W530 with a US keyboard layout has a print screen button between the
right Ctrl and Alt keys, so this happens to me frequently when using the
built-in keyboard. The T420 has a much more sane keyboard (at least, the US
layout, not the Canadian layout,) so I doubt it is that.

What is the model of the external keyboard? It may have a built-in hotkey
for printscreen that you are not aware of.
