Standard questions:
1) Operating system?
2) Output of 'clamav-milter --version'?
3) What changes have you made to sendmail.mc, and what happend when you rebuilt 
sendmail.cf?
4) Any output in /var/log/messages and /var/log/maillog?
5) What runtime options are you giving when you start clamav-milter?
6) Is clamd running?
7) What configuration did you do to clamav.conf?
