The *typical* way this is done is to install a Unix (Linux/*BSD/Solaris) box in front of the Exchange server. This box runs clamav, does RBL filtering, and often runs SpamAssassin as well to filter spam. You point your primary DNS MX at this filter box, which then passes on any valid (not virus, not spam) mail to the Exchange server.
