3.5.9 is obsolete. Upgrade to 4.x immediately (CGP upgrade is free).
You might have to rewrite your web interface (if you already customize it),
but other than that the upgrade process is painless.



Updating is in my experience not always that easy and in the past
several times had strange side effects. Before updating a server I like
to test that first, but right now I miss my backup server I used for
that. So I don't want to update just because of that one new feature.

Because it's not CGP-way. Any content filtering in CGP must be performed
via extfilter interface; no direct modification on the mail spool file.

Is there any other wrapper to ClamAV available? Maybe somebody
wrote a simple script to clamassassin?



None that I know of.
The thing is  ADDHEADER is only available since CGP4.0.
No matter which "glue" you use, you won't get addheader functionality
in 3.5.9
